     To get started, each prospective client is invited to fill out a comprehensive health questionnaire. The information collected is then analyzed by one of our researchers to determine the range of presenting health concerns. A call to the prospective client follows to feed back the initial findings and recommendations for further research. This initial phone contact is also key to establishing a rapport with the prospective client to begin the personalized support that is to be offered and which is a defining quality of care that distinguishes Sky Farm Center. Initial History and Consultation:  $250.00 

Fee Schedule:

      $184. per every four weeks for adults age 21 and over

       $84.00 per every four weeks for children ages 20 and under

Special Note: The above-referenced fees include a weekly phone consultation with one of our researchers to ensure targeted client support as health conditions evolve.
